Classifying Errors in Incorrect Responses

You now have a first set of diagnostic results. The most useful question is no longer simply, “Which answers were wrong?” It is, “What caused each wrong answer?”

That distinction matters because different mistakes need different fixes. If you did not know a grammar rule, you need to learn and practise that rule. If you knew the rule but skipped the word not in the question, revising the rule will not solve the real problem.

In this lesson, you will learn to classify an incorrect response as primarily a knowledge, reasoning, reading, accuracy, or time-management error. You will then apply the system to a few questions from your untimed diagnostic.


Why one wrong answer is not enough information

A score tells you where marks were lost. Error classification tells you how to get them back.

For example, imagine two students both answer the same reading question incorrectly.

  • Student A does not know what the word reluctant means.
  • Student B knows the word but overlooks the sentence beginning with however, which changes the author’s point.

Both lost one mark, but they need completely different practice. Student A needs vocabulary learning; Student B needs more careful reading for contrast words and changes of direction.

This kind of reflection is sometimes called metacognition: noticing how you think, read, and make decisions. It is not about blaming yourself for errors. It is about gathering useful evidence.

The important habit is to avoid vague labels such as “I was careless.” A mistake may look careless, but that label does not tell you what to change next time.

Instead, be precise:

  • “I copied 74 as 47 when writing my working.”
  • “I read least likely as most likely.”
  • “I spent too long on Question 3 and did not reach Question 10.”
  • “I did not know what a dependent clause was.”
  • “I understood the words but made an invalid conclusion.”

Each statement points towards a different repair.


The five error categories

Some study systems use broad labels such as concept, process, and execution. That is a useful starting point, but for your selective-school preparation, we will split errors more precisely into five categories.

In the section “The three-cause model: concept, process, execution,” read the three-cause model. Then, in “How to classify mistakes with examples,” read the concept and process signals. Finish with the execution signals. Focus on the evidence you would see in your own working, not just on the final wrong answer.

Here is the five-category version you will use.

These categories describe the primary cause of an error: the main failure that, if fixed, would be most likely to prevent the same kind of mistake next time.

A question can involve more than one factor. You might rush because you do not know a vocabulary word, then misread an answer choice. However, recording five causes for one wrong answer makes your review confusing. Choose the most useful primary category, then add a short note about any secondary factor.

For instance:

Primary: knowledge. Secondary: time pressure.
I did not know the word scarce, so I spent too long trying to guess its meaning.

The knowledge gap is the first problem to repair. Faster timing will come more naturally once the vocabulary is known.


How to tell the categories apart

The most difficult distinctions are usually knowledge versus reasoning, and reading versus accuracy.

Knowledge versus reasoning

A knowledge error happens when the necessary tool is missing.

Suppose a grammar question asks which sentence has correct subject–verb agreement. You choose an answer because you do not know that the verb must agree with the true subject, not with a nearby noun.

If you could not explain what “subject–verb agreement” means, the primary error is knowledge.

A reasoning error happens when you possess the tool but use it incorrectly.

Imagine these statements:

  1. All city gardens have gates.
  2. The garden beside the library has a gate.

It would be incorrect to conclude that the garden beside the library must be a city garden. You understood both statements, but you reversed the relationship. City gardens are a type of garden with gates; not every garden with a gate is necessarily a city garden.

That is a reasoning error.

A useful test is:

Could I state the rule or idea needed for this question before seeing the answer?

  • If not, classify it as knowledge.
  • If yes, ask whether you used the rule logically and in the correct situation. If not, classify it as reasoning.

Reading versus accuracy

A reading error changes your understanding of what the question or passage says.

Examples include:

  • answering what is most likely when the question asks what is least likely;
  • missing the word except;
  • treating “some” as if it means “all”;
  • overlooking the sentence beginning with but, however, or although;
  • selecting an inference that the passage does not support because you did not return to the relevant lines.

An accuracy error occurs after you have understood the question and selected a sensible method.

Examples include:

  • writing when the question says ;
  • working out the correct answer but clicking or circling a different option;
  • making a multiplication slip after setting up the calculation correctly;
  • leaving out an apostrophe during a rushed final proofread, despite knowing the rule.

A useful question is:

Did the meaning change in my mind, or did my correct plan go wrong in the details?

If the meaning changed, it is usually reading. If the plan was correct but the execution slipped, it is usually accuracy.

Time management is not the same as “I felt rushed”

A genuine time-management error needs evidence about how you used time.

These are time-management errors:

  • spending four minutes on one difficult question and leaving three easier questions blank;
  • refusing to skip a question even when you have no method;
  • rushing the final page because you did not check the clock;
  • not returning to a flagged question despite having spare time.

This is not a time-management error:

“I took a long time because I did not know the fraction method.”

That is primarily a knowledge error. The time loss is a result of the missing method.

Your first diagnostic was untimed, so you should be cautious about using the time-management category for it. You may have taken a long time, but there was no time limit to manage. For the diagnostic, focus mainly on knowledge, reasoning, reading, and accuracy. Timing becomes more important when you begin timed practice later in the course.


A reliable review routine

When you see a wrong answer, do not immediately read the explanation and say, “Oh, that was easy.” Once you know the correct answer, the question often feels much simpler than it felt during the test.

Instead, review it in this order.

  1. Return to your original answer and confidence code.
    Look at what you selected and whether you marked it C, U, or G. A confident wrong answer often reveals a mistaken rule, inference, or reading of the text. An unsure or guessed wrong answer may reveal a missing skill, but not always.

  2. Hide the answer key and retry the question slowly.
    Use no notes, no hints, and no explanation. Write a few words about what the question is asking and show your working or evidence.

  3. Find the first point where your thinking went wrong.
    Do not stop at “my answer was B but the answer is D.” Ask: What made B seem right at the time?

  4. Classify the primary cause.
    Use the five categories. Base the label on evidence from your original working, your memory of the attempt, and your slow retry.

  5. State the error in one precise sentence.
    Avoid “careless” and “I need to focus.” Those are feelings, not diagnoses.

Here are examples of precise classifications:

Notice that each explanation contains something you can observe and change.


Apply the system to your diagnostic

Take out your Diagnostic 1 folder from the previous lesson. Choose three questions to review:

  • one incorrect question you marked C;
  • one incorrect question you marked U or G;
  • one further incorrect question from either Reading or Mathematical Reasoning.

If you have fewer than three incorrect answers, include a correct answer marked G. A lucky correct answer is not yet a dependable strength.

For each chosen question, make a temporary review table in your notebook:

Your evidence should be specific. For example:

Question: Reading 4
Original thought: “Option C sounded sensible.”
Slow retry: “The passage says the character was relieved, but Option C says he was disappointed.”

Evidence: “I chose an answer from memory instead of returning to the sentence that described his reaction.”

Or:

Question: Mathematical Reasoning 7
Original thought: “I divided by 3 because there were three groups.”
Slow retry: “The question asks for the total of three equal groups, so I needed multiplication.”
Primary error type: Reasoning
Evidence: “I knew multiplication and division facts, but I chose the wrong operation from the wording.”

Do not worry if you are uncertain between two categories at first. Write your best classification and explain your evidence. The explanation is more valuable than the label alone.

A helpful final check is this:

If I met a similar question tomorrow, what single improvement would make me more likely to get it right?

If your answer is “learn the rule,” it is probably knowledge. If it is “choose a better method” or “justify the inference,” it is probably reasoning. If it is “read the command word and find the evidence,” it is probably reading. If it is “check my transfer or arithmetic,” it is probably accuracy. If it is “skip earlier and return,” it is probably time management.
